Nurse Practitioner (NP) / Physician Assistant (PA) - Long-Term Care

A specialized vascular care group dedicated to serving the long-term care setting is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its growing team. The organization delivers advanced, clinic-level vascular care directly within nursing homes, allowing patients to receive high-quality treatment in a familiar environment.

This role involves providing personalized bedside care, reducing the need for patient transport while ensuring comprehensive vascular services-from routine assessments to advanced procedures.

Position Overview:
The NP/PA will perform on-site clinical assessments and develop appropriate treatment plans for residents in long-term care facilities. This position reports to a Lead Nurse Practitioner and operates within a mobile practice model.

Schedule:
Full-time (40 hours per week)

Key Responsibilities:
  • Conduct on-site clinical assessments and complete comprehensive evaluation documentation

  • Perform detailed histories and physicals for acute, chronic, and complex patients

  • Order and interpret diagnostic and therapeutic tests based on patient needs

  • Deliver care in accordance with facility, state, and federal regulations

  • Monitor and evaluate treatment effectiveness

  • Build and maintain professional relationships within assigned facilities

  • Educate patients, families, and facility staff on services and care plans

  • Support development of provider and facility partnerships as assigned

  • Complete all required reports accurately and in a timely manner

Qualifications:
  • Bachelor's degree required

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program

  • Current NP or PA license

  • BLS (Basic Life Support) - required

  • ACLS (Advanced Cardiac Life Support) - preferred

  • Current Infection Control Certification

  • Board Certified or Board Eligible

  • Required health documentation: PPD/TB, Hep B, MMR, Varicella

  • COVID-19 and influenza vaccination preferred

  • Minimum 1 year of NP/PA experience preferred

  • Reliable personal transportation required

Preferred Experience:
  • Experience in nursing home or long-term care settings

  • Background in outpatient vascular care

  • Ability to work independently in a fast-paced environment
